opinions needed... definitive bezel insert chart

It seems like it is a fairly regular occurrence to see bezel insert size posts. People have put together charts and posted sizes here and there, but it has never been an organized project.
This is where I can help, if there is enough interest. I use Google Forms quite regularly. If I put together a Form, people can submit the information and it will automatically be added to a spreadsheet that I can share the link to view. Ideally I see this as a stickied post that contains two links, one to submit insert dimensions, and one to view the resulting spreadsheet. This part obviously isn't up to me, but if the interest level is high enough I'll try to get this all put together. Please share your thoughts and feel free to contribute ideas as well.
Right now I'm thinking the resulting spreadsheet would be divided into pages based on brand and within that it would list model, nickname (if applicable), inner diameter, outer diameter, and an optional comment section.
This would eliminate the problem of a shared spreadsheet that can be messed with, because the resulting spreadsheet would be view only.
